Once you are booked…

4 Steps to take ahead of your first visit!

Step 1

Gather your Extended Health Benefits Information

We do direct billing...

but don’t leave yourself any billing surprises as you are responsible for the full payment when you come in if your company is experiencing any system glitches!  Call your company and fill in this form ahead of time to speed up your billing process to maximize your health benefits.

Step 3

Sign into your secure online records portal

Follow the link in the appointment confirmation email, there will be a link to New Client Forms

Check your email for an invitation to log in to your ChARM Patient Portal 

Save your login information

You will receive an email with practitioner forms – please follow the instructions to fill out your questionnaires

Step 4

Arrive 20 Minutes Early

.... for your first visit to the clinic

To clear up any billing questions, to sign any missing forms so that we can start your visit on time!

Cancellation Fees, Payment & Direct Billing Policies

If you have to cancel within 24 hours (or 1 business day) of the appointment start time, you will be charged the full visit fee.
NOTE: we are closed on Sunday, so all Monday cancellations must be made by Saturday!

Empower Health cannot guarantee that your insurance company will accept Direct Billing under all circumstances, so you will need to have a backup payment method.

Sign in to see your Visit Notes and follow-up instructions from your practitioners.  ( For video instructions click HERE ) If you did not receive an email invitation to log in within 24 hours of your visit, please call 604-336-2844.
